Your Privacy and Confidentiality
Chiropractic Works FNQ Pty Ltd is committed to complying with the Privacy Act 1988 and the Australian Privacy Principles 2014 and the privacy provisions of all applicable legislation.
We will only collect personal information, do it fairly and as necessary to assist in our services and functions.
We will advise individuals of the purpose for which their personal information is collected.
If we collect sensitive information (as defined under the Act), we will treat it with the utmost security and confidentiality. We will ensure that it is not collected for any purposes, other than those for which we have obtained the individual’s consent, unless the law requires otherwise, or other exceptional circumstances prevail as described under the Act.
Where an individual chooses not to provide requested information, we will advise that individual of what consequences this non-disclosure may have. For example, withholding certain information may limit our ability to provide relevant offers or services to individuals.
Your privacy at Chiropractic Works is very important to us. Know that nothing about your care is released to any third party without your prior consent or knowledge. If you are moving away, we will ask you to sign an Authority to Release Information Form to aid in the transfer of a summary of your records. Your details may only be requested to be released by you, or another medical practitioner or clinic. You will always be contacted if a request for information is made.
From time to time, practitioners within our office will discuss cases to assist each other in ensuring you are getting the best care from their combined knowledge. Cases will be de-identified before discussion and nothing about your care will be discussed with any other patient in our clinic.
The information that is kept on file for our patients is as follows:
- Name, date of birth, address, contact details, including an emergency contact
- Medicare and DVA details, health insurance details where relevant
- Transaction records (only the amount and how it was paid, no credit card details kept on file)
- Any paperwork that you have completed to be a patient of our clinic
- Any reports/documents you have given us from other practitioners
- Any imaging we have taken in our clinic
- Any imaging you have brought in to our clinic
- Clinical notes from consultations with our practitioners
- Any letters sent from our clinic to other practitioners
- Any letters received from other practitioners
- Any letters/documents received from third party insurers, Workcover or Lawyers
Your contact details will be used for in-house communication from us to you regarding your appointments, follow up care and marketing (in-house only). If you do not want to be included on any in-house marketing emails/sms, please let us know.
Your transaction records (i.e hicaps/eftpos slips) are stored in a secure storage room and destroyed after 7 years. No credit card details are kept on your file.
All of our documentation is stored electronically on well protected, encrypted and backed up servers. Any paper documents are scanned and stored electronically, with original copies of these documents stored in a secure storage room.
A copy of your records can be made available to you at your request, with a signed Authority to Release Information form.
If you have not attended our clinic for more than 10 years, any paper documentation is destroyed, and any electronic file is archived. This is also done 10 years after a pediatric patient turns 18.
Our records and database are not available for sale to third parties for any purpose.
As Chiropractic Works FNQ Pty Ltd is committed to protecting the privacy of individuals, we will view unauthorised disclosure of, or access to, personal information by our employees or contractors, as a serious breach of this policy. Appropriate action (which may include disciplinary or legal action) will be taken in such cases.
